Registration for the Settlement Program opened on February 6, 2017!

The In re: National Football League Players’ Concussion Injury Litigation class action settlement became final and effective on January 7, 2017, which is the “Effective Date” of the Settlement. For more information about the Settlement Agreement, click here to read the Frequently Asked Questions.

It is time to register with the Claims Administrator!

REGISTRATION IS OPEN!
You must register with the Claims Administrator to later receive any benefits.
You must register even if you had “signed up” for more information.

All Settlement Class Members must register with the Claims Administrator to receive any Settlement benefits. The deadline to register is August 7, 2017. Settlement Class Members include Retired NFL Football Players, authorized representatives of deceased or legally incapacitated or incompetent Retired NFL Football Players (“Representative Claimants”) and family members or other persons who properly under applicable state law assert the right to sue independently or derivatively by reason of a relationship with a Retired NFL Football Player (“Derivative Claimants”). (Derivative Claimants also will be permitted to register within 30 days of the Retired NFL Football Player’s submission of a Claim Package, but we urge all Settlement Class Members to register as soon as possible.)

You can register in one of two ways:

1.Online: To submit a Registration Form electronically, click the REGISTER NOW button below. Then select what type of Settlement Class Member you are and follow the instructions provided. This is the fastest way to register.

2.Hard Copy: Click here to download a hard copy Registration Form and click here for instructions on how to complete the Registration Form. After printing and completing the Registration Form, you can send it to the Claims Administrator by:

THE CLAIMS PROCESS IS OPEN FOR REGISTERED SETTLEMENT CLASS MEMBERS!

Settlement Class Members who have registered and who have received a favorable Registration Determination Notice from the Claims Administrator now can submit an MAF Claim Package or a Derivative Claim Package if the Retired NFL Player has been diagnosed with a Qualifying Diagnosis (for more information on those conditions, click here). The easiest way to do that is online. Click here to log in to your Portal or set up a Portal with the Claims Administrator and then follow the instructions there on how to submit your Claim Package online.

You also can submit an MAF Claim Package or a Derivative Claim Package by mail. Click here to get a PDF of the Claim Package Forms you need, or call 1-855-887-3485 or email ClaimsAdministrator@NFLConcussionSettlement.com to ask that the forms be mailed to you.

You cannot submit a claim unless the Claims Administrator has notified you that you are properly registered in the Program. On March 23, 2017, the Claims Administrator started issuing notices to those who have registered. Call, email or write the Claims Administrator if you have questions.

THE BASELINE ASSESSMENT PROGRAM WILL OPEN ON JUNE 6, 2017

All Retired NFL Football Players with at least one-half of an Eligible Season and who timely register in the Settlement will be eligible to receive a baseline assessment examination in the Baseline Assessment Program, which opens on June 6, 2017. Starting on May 8, 2017, eligible Retired NFL Football Players will be able to schedule an examination by contacting the BAP Administrator and will receive additional information on how to do so in their Registration Determination Notices.
